Medical Action notified customers of an urgent product recall from Triad Group involving all lots of alcohol prep pads, alcohol swabs, and alcohol swab sticks issued on January 3, 2011.

The recall applies only to the following Triad Alcohol products within kits and trays, NOT to kits and trays themselves.

11-SA03 Sterile Alcohol Swab x 3
PL-3536 (MAI# R0878) Alcohol Medium Prep Pad

This issue does not affect the integrity of the remaining components in the tray so clinicians may safely use the kit once the Triad alcohol components are removed. End-users are instructed to remove and discard the Triad alcohol pads and/or swabs in the trays when opening the affected trays. Any other brand of pad or swab in the tray is safe to use.

To minimize disruption to the healthcare industry, we are performing a field correction rather than requiring that kits be returned. We are instructing distributors and end-users to label affected kits and trays for the clinicians. The label instructs the healthcare professional to discard any Triad alcohol products from the Medical Action tray upon opening. Triad products included in Medical Action kits other than the ones listed above are not part of the recall and are safe to use. Discarded alcohol product can be destroyed and documented on the form on this page.
